	New Appointments / Consultations

	Patients desiring allergy testing require a referral from a physician. Appointments are scheduled and the patient is notified by both the referring physician's office and the clinic's automated phone messaging system of the appointment's date and time.

	Assessments

	Patient re-assessments are done on an annual basis and are usually scheduled after the first visit. If a patient is not re-assessed for a period exceeding two years, a new consult letter from a referring physician is required.

	Follow-up Visits

	Follow-up visits are scheduled at the discretion of the Allergist and may occur in periods less than one year.

	Challenges

	Food, drug and local anesthetic challenges are scheduled as needed. The process requires the patient to be present in the clinic for a minimum period of four hours and therefore are only scheduled for 8:30 AM on the day of the appointment.


		Missing a Scheduled Appointment

		Missing a scheduled appointment is a discourtesy to both the Allergist and to other patients awaiting an appointment to see the Allergist. A NO SHOW fee of $100.00 is billed to all patients missing a scheduled appointment and the outstanding account will need to be settled before an appointment is rescheduled.

	Please DO NOT attempt to cancel an appointment by EMAIL. The latter may not be checked for up to 4 weeks. Please call the office to cancel.


	IMPORTANT NOTE: No anti-histamines are to be taken 48 hours prior to a scheduled appointment. Inhalers and puffers are allowed.

Penicillin/Amoxicillin Testing: The Prepen Reagent used for Penicillin/Amoxicillin testing is NOT covered by OHIP. There is a $70 fee for the reagent which may be covered by drug plans. DIN: 00328693
